Difficult Hysteroscopy Entry Hypertrophied Crypt
This educational video presents a rare and instructive case of primary infertility caused by a hypertrophied cervical crypt leading to repeated failure of HSG, IUI, and cervical dilatation. The lecture discusses the differential diagnosis of difficult cervical entry before demonstrating hysteroscopic evaluation using a 2.9 mm hysteroscope. A bipolar resectoscope is then used to carefully shave the hypertrophied cervical crypt, restoring a clear passage through the cervical canal while preserving normal anatomy. The video also compares bipolar and monopolar resectoscopic techniques and explains important technical considerations during surgery. The successful postoperative outcome demonstrates how recognition and treatment of this uncommon pathology can simplify future fertility procedures.
