Lap Myomectomy Broad Ligament Fibroid
This advanced laparoscopic surgery video presents the management of a rare true broad ligament fibroid in a 30-year-old woman with an incidentally detected pelvic mass. Preoperative ultrasound and MRI suggested either a broad ligament fibroid or an ovarian cyst because no communicating vascular pedicle with the uterus could be identified. The surgeon demonstrates systematic ureter identification, limited ureteric dissection, vasopressin-assisted hydrodissection, and meticulous capsular enucleation using the correct anatomical planes. Throughout the procedure, emphasis is placed on protecting the ureter while achieving a nearly bloodless dissection. The surgery concludes with confirmation that the fibroid has no uterine attachment, establishing the diagnosis of a true broad ligament fibroid.
