Laparoscopic Uterine Artery Ligation at Origin (TLH)

This video provides a detailed demonstration of laparoscopic uterine artery ligation at its origin, a technique commonly used during difficult hysterectomies and myomectomies. It begins with adhesiolysis and preparation of the pelvic sidewall before entering the retroperitoneal space. The surgeon explains how to identify key anatomical landmarks, including the external iliac vessels and ureter. The dissection then progresses through the pararectal space to locate and isolate the uterine artery. Special emphasis is placed on distinguishing the uterine artery from the uterine vein and protecting the ureter throughout the procedure. The video concludes by demonstrating safe bipolar coagulation of the uterine artery and discussing principles of hemostasis and ureteric safety.
