Massive Broad Ligament Fibroid: Open Hysterectomy
This video demonstrates the open surgical management of a giant right-sided broad ligament fibroid measuring approximately 17–20 cm.
Preoperative imaging showed a massive parauterine mass, and the surgeon explains why open surgery was considered more appropriate than laparoscopic or robotic surgery in this particular case.
The fibroid is delivered through a Pfannenstiel incision, allowing excellent exposure despite its large size.
Special emphasis is placed on ureteric safety, with the surgeon staying close to the fibroid and pushing the broad ligament peritoneum away during enucleation.
A myoma screw, capsular incision, traction and finger dissection are used to progressively mobilize the mass.
The remaining pedicle is then divided, and both ureters are inspected for visible peristalsis.
The video provides important lessons on selecting the appropriate surgical approach for very large pelvic masses.
