Saving The Ureter in Laparoscopic Hysterectomy (TLH)
This advanced laparoscopic surgery video demonstrates the prevention of ureteric injury during total laparoscopic hysterectomy in a patient with unsuspected pelvic endometriosis. Although preoperative imaging suggested a routine hysterectomy, intraoperative findings revealed a thickened uterosacral ligament and distortion of the normal ureteric pathway. The surgeon performs retroperitoneal dissection and ureterolysis to identify and separate the ureter from surrounding fibrotic disease before proceeding with hysterectomy. Detailed explanations of pelvic anatomy, ureteric peristalsis, uterine artery relationships, and energy safety are provided throughout the procedure. Special emphasis is placed on recognizing altered anatomy and adapting surgical strategy accordingly. This is an excellent educational resource for gynecologic endoscopic surgeons seeking to reduce the risk of ureteric complications.
